There's a sink on top of the toilet because the sink uses the water that goes into the toilet's tank instead of water separate from the toilet. Since the water goes into the sink before going into the tank, the only water the toilet uses to flush is water that has already been used to wash your hands. It's a thing they came up with to use less water.

Also, it's plugged in for the toilet seat. It likely has a bidee, heating, etc.
