Surgeons have gotten a lot better these days. The results can look pretty darn close to cis-fem equipment, if they've done their job well.

There are still some serious pitfalls with bottom surgery though. Especially if someone has been on HRT from a young age and thus has a very small penis. If you don't have enough penile tissue they use a portion of your colon to complete the vagina... which can lead to a constant smell of shit coming from the hole. Even not including the colon bad smell and infection are common issues. So I definitely understand trans women choosing not to do bottom surgery.
