That it's gore and that the artist choose to put it off-screen? The angle is from the guys head looking inwards over his body. The pins are about chin-height. She's stabbing the floor either side of his head to stop him from escaping. The intent is a femdom/threat sort of play. If the intent was gore here, there would be any amount of blood shown.
