That's a fan scene for TLOU 2. She's 19. In this version, she's not lesbian.

Joel knows that Ellie is NOT Sarah, and grief for her is destroying him. He wants to end it. He asks Ellie her opinion.

Ellie is 19 and has perfectly normal feelings of wanting to fuck and be in love with Joel forever. But she has to hide that because she knows that he only sees her as a poor substitute for his daughter.

The daydream while she drives is triggered by his asking her to help him die. It's all the things she wants to do with Joel that she realizes now will never happen.

Wanting to love and fuck Joel is a "filthy secret" she keeps from him. And it tears her apart, just like memory of Sarah tears Joel apart.

Ellie says "do it" and holds the gun for him, both to help him end his suffering, and to end hers—her knowing that he will never love her as the woman she has become.

That was BEAUTIFUL.

But why was she naked? Maybe after agreeing to hold the gun, she told him how she feels about him, about her love. She asked him to fuck her first, before he dies, so she can at least have the only thing she wants in her life before she dies too, someday ...even if it's only once.
