Anon 1 they are all pregnant, Shizuka being the largest woman of the group would naturally be the first to 'show her secret' whereas the much slimmer Rei and Saya appear to have 'average girl' waistlines. The tell is how all their breasts are so full of milk that they have a "mound of venus" shape despite not having any sort of support. (Saeko's right breast being the only exception)
