In VERY simple terms: the nipple has `lobules` connected by a network of thin tubes, known as `ducts`. When breastfeeding or other methods, ducts carry milk from the `alveoli` to the areola (the ring/circle around the nipple). From the areola, the ducts join together into larger ducts ending at the nipple. This is not how it works, it's not completely hollow (although it is technically hollow due to what I just said) Breasts are not self-lubricating as this image suggests.
