I think what he intends to mean is that asshole = box. And that if you got in the scene from the perspective of caitlin cooke, then you could see the other woman's asshole (box).
To try to communicate that with telling people to turn a picture around is very poor communication skills. He should have said turn the hypothetical scene around, the picture is just the 2d image we are looking at.

Because if you rotate this picture any way or flip it vertically or horizontally you don't get anything that resembles a lipstick covered box.
